Aware Recovery Care Maine implements an innovative “Recover Where You Live” model for treating addiction in adults aged 18 and older. Their in-home addiction treatment (IHAT) program uniquely integrates both in-home and virtual services, tailored to individual preferences through concierge options. Aware Recovery Care employs a comprehensive biopsychosocial approach that includes evidence-based practices, medication-assisted treatments (MAT), individual and family therapy, and organized care in collaboration with external healthcare providers. The foundation of Aware Recovery Care’s treatment philosophy lies in compassion and understanding, emphasizing that every person has the right to pursue sustainable recovery. By focusing on the specific requirements of each client and offering persistent support within their living environment, they aim to lead individuals towards healthier, more fulfilling lives free from addiction. Every client is paired with a care coordinator (either a nurse or social worker) along with two client recovery advisors (CRAs) who provide peer support and help navigate the treatment process. They assess necessary external care and facilitate arrangements with primary care physicians (PCPs), individual therapists, medical experts, and family education facilitators. Aware Recovery Care’s medical director also coordinates with existing healthcare professionals if clients are already engaged with them. A family education facilitator works closely with family members, offering insights on addiction and recovery to foster collective healing. The program at Aware Recovery Care is flexible, extending from one month to a year, with clients receiving up to 20 virtual one-on-one therapy sessions with an Internal Psychotherapist each month. During these sessions, they focus on stress management, enhancing self-esteem, boosting self-compassion, and refining communication skills, alongside addressing and reframing detrimental thought patterns. On a typical day at Aware Recovery Care, clients experience a one-hour visit from a care team member, along with either an in-person interaction from one of the two CRAs or a video session with their dedicated therapist. Clients also have monthly meetings with their care coordinator, ensuring that the care team regularly collaborates to align treatment with clients' evolving needs and objectives. As clients approach the conclusion of their program, Aware Recovery Care implements a customized discharge process that aligns with individual treatment plans and living situations. The clinical coordinator engages clients in discussions about their comprehensive discharge strategy, which includes maintaining connections with social support, outlining future goals, and scheduling aftercare appointments with licensed professionals involved in their recovery journey.
The primary focus of this center is to address issues related to substance use disorders, assisting individuals in achieving stability, developing relapse-prevention strategies, and linking them to empathetic support systems.
