Aware Recovery Care Massachusetts utilizes an innovative “Recover Where You Live” model for treating addiction in adults aged 18 and older. Their in-home addiction treatment (IHAT) program combines personalized in-home support with virtual care and concierge services tailored to individual circumstances. Aware Recovery Care employs a biopsychosocial framework that includes evidence-based methods, medication-assisted treatment (MAT), individual and family therapy, alongside coordinated care from other healthcare providers. The center's treatment philosophy emphasizes empathy, patience, and the conviction that each person is entitled to a chance for sustained recovery. By focusing on the distinct requirements of each individual and providing ongoing support within their home environment, Aware Recovery Care aims to help clients lead healthier, more fulfilling lives free from addiction. Every participant is assigned a care coordinator—either a nurse or a social worker—and two client recovery advisors (CRAs) who serve as peer supporters throughout the recovery process. These professionals identify any necessary external care and set up appointments with primary care doctors, individual therapists, medical specialists, and family education facilitators. Aware Recovery Care’s medical director also coordinates care with any existing healthcare providers the clients might already be working with, while a family education facilitator offers support and information to loved ones about addiction and recovery. The program at Aware Recovery Care can last from one month to a year and is updated monthly. It includes up to 20 virtual one-on-one therapy sessions with an Internal Psychotherapist. During these sessions, clients focus on stress management, enhancing self-esteem, self-compassion, and communication skills, while also addressing and transforming negative thought processes. A day at Aware Recovery Care typically features an hourly visit from a member of the care team, along with either an in-person visit from one of the CRAs or a virtual meeting with their one-on-one therapist. Clients also have monthly check-ins with their Care Coordinator. The care team engages in ongoing communication to ensure the treatment remains aligned with the clients’ evolving needs and objectives. When clients are ready to transition out of the program, Aware Recovery Care implements a customized discharge process that reflects their specific treatment plans and home situations. This involves discussing a thorough discharge plan with their clinical coordinator, which outlines continued engagement with their social networks, established objectives, and aftercare appointments with licensed providers from their previous treatment.
