Project Transition provides support for adults aged 18 to 65 experiencing mental health challenges, including those with co-occurring issues such as substance use disorders, developmental disabilities, or physical health ailments. Located in Tennessee, they offer both residential and day treatment options within apartment settings. The Nashville facility is situated at Haven Apartments, previously recognized as Woodbridge Apartments. A distinctive feature of Project Transition is its strong focus on fostering a therapeutic community within the mental health domain. The center opts to refer to individuals undergoing treatment as "members" rather than "clients" or "patients," fostering a sense of belonging and facilitating a move towards integration into the community. Members are encouraged to engage in an apartment complex community, aiding their re-entry into the larger society. This approach aims to reduce stigma and promote normalized social interactions. Project Transition employs dialectical behavior therapy (DBT) with a personalized, strengths-based focus in its mental health programs. The dedicated staff, comprising approximately 30 professionals, including certified peer and recovery specialists, provide ongoing support throughout the day. Their approach involves using evidence-based therapeutic practices tailored to the unique goals of each member. Staff are available from 7 a.m. to 9 p.m., with options for members to reach out for emergency assistance during off-hours. Throughout the day, staff at Project Transition conduct workshops on skill-building, manage medication, provide therapy sessions, and facilitate group meetings. Individual therapy sessions, psychiatry consultations, and group therapies occur biweekly, while the other days involve community gatherings, skill development workshops, goal-setting exercises, and meetings for Alcoholics Anonymous (AA) or Narcotics Anonymous (NA). An onsite driver is available to escort members to off-site AA or NA meetings and medical appointments. Furthermore, Project Transition has a support system in place for children and adults experiencing behavioral health issues that affect their communities. They collaborate with crisis intervention teams and family support networks to minimize the need for hospitalization and mitigate crises. The center outlines 13 criteria that serve as benchmarks for member success. Family and friends are welcome to visit during designated hours with the consent of other housemates, but overnight stays are not permitted. Project Transition also provides partially furnished apartments, allowing members to personalize their living spaces.

At our center, we collaborate consistently with many of the major organizations in the regions we operate, such as Community Care, United Healthcare, BlueCare Tennessee, and Community Behavioral Health. We are open to referrals from all private insurance providers and will actively seek out single-case agreements or options for private payment.
