Aware Recovery Care Connecticut employs a distinctive “Recover Where You Live” method designed for adults aged 18 and over dealing with addiction. Their innovative in-home addiction treatment (IHAT) program merges in-person and virtual services while offering concierge assistance to cater to individual preferences. Aware Recovery Care takes a comprehensive approach to addiction by utilizing a biopsychosocial model that integrates evidence-based practices, medication-assisted treatment (MAT), individual and family therapy, as well as coordinated services from other healthcare providers. Should the need arise, clients may participate in medically supervised detox managed through virtual nursing and MAT support. The foundational belief of Aware Recovery Care is rooted in compassion, empathy, and the conviction that everyone deserves a shot at sustainable recovery. By focusing on the specific requirements of each client and providing ongoing support in a home setting, they aim to lead individuals toward a healthier, more satisfying life free from addiction. Each participant in Aware Recovery Care is assigned a dedicated care coordinator—either a nurse or social worker—and two client recovery advisors (CRAs) who serve as peer supporters throughout the recovery journey. They assess any additional care needs and facilitate appointments with primary care providers (PCPs), therapists, medical specialists, and family education facilitators. If clients are already connected with these professionals, Aware Recovery Care’s medical director works to ensure seamless coordination of care. The family education facilitator offers support and valuable insights to family members about addiction, recovery, and collective healing. The program at Aware Recovery Care is designed to extend monthly, with treatment durations ranging from one month to a full year. The hybrid in-home and virtual services include as many as 20 individual virtual therapy sessions with an internal psychotherapist. During these sessions, clients focus on managing stress and enhancing their self-esteem, self-compassion, and communication abilities, while also working to transform negative thought patterns. A typical day at Aware Recovery Care features an hour-long meeting with a care team member, along with either an in-person visit from one of the CRAs or a virtual session with a therapist, depending on the day's schedule. Clients also have monthly check-ins with their care coordinator. The care team continuously communicates to ensure that treatment remains aligned with the clients' evolving needs and goals. The discharge process at Aware Recovery Care is customized to fit each client’s specific treatment plan and home circumstances. They engage in discussions about a detailed discharge plan with their clinical coordinator, which encompasses ongoing participation in support networks, the objectives they've set, and aftercare appointments arranged with licensed professionals involved in their treatment.
